If our review of your cases reveals the absence of a required document, we will ask you to produce it from the original case file. If you are unable to produce the document, we will require you to submit a fresh case list. The only document you may legitimately create after the fact is the required rationale for surgery statement if one does not exist in the original case file. You may not create documents after the fact to rectify any other type of missing data. If documentation for one of your cases contains date errors, such as a pathology report date that is inconsistent with the surgery date, we will ask for an explanation. If your explanation is not satisfactory, we may require you to submit a replacement case.
